Collection notice – How we use your information
The information you provide in a complaint is collected by the Department of Government Services (DGS) on behalf of First Peoples - State Relations Group (FPSR) in the Department of Premier and Cabinet. All personal information is held, used, managed, shared, disclosed and stored by FPSR in accordance with the principles in the Privacy and Data Protection Act 2014. FPSR stores your complaint, including any accompanying attachments, on a secure database that is only accessible to the relevant Director and designated staff in FPSR. We do not use or disclose your personal information for purposes other than what it was collected for, unless permitted or required by law.
DGS discloses your information in the complaint form to FPSR for the purpose of enabling your complaint about the conduct of a heritage advisor to be considered. This can include personal information about you, including your name and contact details, and your information regarding the alleged misconduct of a heritage advisor. The nature of complaints may also inform improvements to FPSR’s policies, processes and guidance materials.
By providing the information requested in the complaint form, you are giving consent for FPSR to collect this information. If you do not wish to provide the information requested, FPSR may not be able to fully consider your complaint. You may make an anonymous complaint, but unless you provide contact details, this will limit how we can proceed, such as not being able to get in touch with you for more details or letting you know the outcome of your complaint.
DGS uses the services of Drupal 9 to administer this form. The information that you provide is stored by DGS on the Microsoft Azure infrastructure in Sydney, Australia.
